When a tooth has been severely damaged from decay or fracturing and can not be supported by a filling an Onlay or a Crown is indicated. A good rule of thumb is if a tooth has lost 50% of its original structure a tooth will not be strong enough to support a filling. Crowns are restorations that cover the entire tooth essentially replacing all of the enamel. An Onlay is like a partial crown, only replacing the part of the tooth that has been destroyed. The amount of healthy tooth structure remaining after removing decay is the deciding factor whether a Crown or an Onlay is placed. Bridges are prosthetic devices that help us to replace missing teeth in a way that is permanent and non-removable. By crowning the teeth next to the space a false tooth can be attached to the crowns giving an appearance that the tooth was never missing.

When enough tooth structure remains after the removal of old silver fillings and/or decay a composite restoration can be placed. This is a material that has been developed over the years as an esthetic replacement for amalgam/mercury or silver filling material. Amalgam/mercury fillings do not behave like a tooth. When you put something cold in your mouth your tooth shrinks and when you put something hot in your mouth your tooth will swell. Amalgam fillings also do this but they do it 10 times faster than the tooth. This allows a gap to widen between the tooth and the filling. Where there is a gap, there is a huge area for bacteria to get under the filling and cause recurrent decay. Another issue with the amalgam is that as the material expands and contracts it is causing microfracturing in the tooth structure. Eventually these fractures widen and become large cracks in the tooth allowing more bacteria admittance. Composite resin is almost identical to the tooth in its rate of expansion and contraction on thermal changes. This decreases the chance of a gap opening between the tooth and the filling. It can be bonded to the tooth, and it is much more natural in appearance. Composite resin fillings are more technique sensitive than placing amalgam but when placed correctly will give many years of service.

Preventive Dentistry:

Daily preventive procedures help remove bacterial plaque from the mouth. Bacteria eat the sugars we eat and then excrete acid which causes both gum disease and decay. Plaque is a combination of bacteria, which are very sticky, and food debris (particularly the sugars we eat). When the plaque is not removed in a 24 hour period it soaks up the minerals in our saliva and becomes hard like cement. This is called calculus and cannot be removed by normal brushing and flossing. Calculus is also very porous giving the bacteria a protected haven to stay on your root causing gum irritation and infection. Unfortunately, no matter how good you are at your daily oral hygiene plaque will be missed and calculus will form. This is the reason we recommend routine professional cleanings with our hygienist. Regular cleanings will keep your mouth healthy and your breath fresh.

Periodontal Therapy:

Infection of the gums and supportive tissues of the teeth can be controlled and in some cases reversed. When the infection goes on unchecked, recession of the gumline and pocketing in the bone results. In some cases the destruction is so bad that teeth will be lost. One of the major problems with periodontitis is that it is a painless disease that can only be diagnosed by your dentist. When pain finally occurs it is often too late and severe bone destruction has already occurred. Prevent future tooth and supportive tissue loss with effective gum treatments.
